What are the Expected Outcomes of Breast Implant Revision?

The outcomes of breast implant revision can be analyzed in three main categories: aesthetical, psychological, and health-related.

Aesthetical Outcomes

If you chose to have a breast implant revision because you are not happy with the size or the shape of your previous implants, you will obtain the breasts that look the way you desire. How Is A Breast Implant Revision Done?

Breast implant revision is a cosmetic surgery procedure performed for various reasons. It is customized according to the needs and wishes of the patient.

Anesthesia

Like every other surgery, breast implant revision surgery requires anesthesia. Your surgeon will decide if general anesthesia or local anesthesia with sedation is the more suitable one for your case. But typically, general anesthesia is used in breast implant revision surgeries.

Surgery

There are different reasons to have a breast implant revision surgery, including the desire to remove breast implants, or to replace the implants with different ones (different size, shape, or type); reversing the effects of aging, weight fluctuations, or pregnancies; and treatment for capsular contracture. Therefore, the details of the procedure depend on the needs and the goals of the patient. The surgery takes 1 to 3 hours, depending on the specifics of the procedure.

Incision

In most of the cases, the same incisions used in the original breast augmentation surgery is used. However, if there is a need for a breast lift, other incisions may be required. The exact pattern of the incisions depends on the amount of lift that is required. A small incision at the top of the areola, or an incision around the areola with a vertical incision extending down to the natural crease below the breast, may be necessary. In severe cases, a horizontal incision along the crease below the breast may be added.

Breast implant revision has several approaches to each different conditions.



Changing the Size or the Shape

You may choose to have a revision surgery because of being not content with the size or the shape of your breasts. In that case, your surgeon will remove your previous implants and manipulate the space inside your breasts to fit the new implants. If your new implants are larger, they may enlarge the implant pocket. If your new implants are smaller, they may need to reduce the size of the implant pocket with sutures. Then they will place your new implants. It may be necessary to use an Acellular Dermal Matrix (a type of surgical mesh) to add support.

Treatment for Capsular Contracture

Capsular contracture is a painful and discomforting complication caused by a tightened and hardened tissue capsule that is squeezing the implant. During the breast implant revision surgery, this issue is addressed by your surgeon removing the old implants and the hardened tissue capsule, then inserting your new breast implants.

Revision with a Breast Lift

Whether it is because of aging, weight fluctuations, or pregnancies; you may end up having sagging breasts. This can be corrected through a breast implant revision in combination with a breast lift. In this procedure, in addition to replacing your implants, your surgeon will remove the excess skin from your breasts and reposition your nipples and areolas.

Treating Implant Rippling

Sometimes, the edges of saline implants become visible, and they can be felt. There are several options to correct this problem: the implants may be repositioned, a new implant can be placed in a different implant pocket with thicker muscle coverage, other tissues can be used to cover the edges of the new implant, or a different type of implant can be used.

Correcting Implant Malposition

During the original breast augmentation surgery, the implant pockets may have been created too close to or too away from each other. In that case, your surgeon will reconstruct the implant pockets in the appropriate positions by manipulating the scar tissue in or around the implant pockets. Sometimes, it may be necessary to use an Acellular Dermal Matrix (a type of surgical mesh) to add support to the newly positioned implant pockets.

Implant Removal

You may want your implants to be removed and no new implants to be placed, because you don’t want them anymore. Maybe, if your implants were large, they may have been causing sagging in your breasts, and you may want them to be removed to correct this issue. A breast lift may be necessary to correct this problem, but sometimes the removal of the implants is sufficient. During your breast implant removal procedure, your surgeon will remove your implants and the tissue capsule formed around them.

Breast Implant Revision is either done in order to prevent implant-related complications, or as a result of some complications. The complications are the same as the breast augmentation with implants, as the procedure is related. Although, if the implant is removed, implant-related complications will be removed as well.

What To Expect During A Breast Implant Revision Recovery?

The First Week

In the majority of the cases, the recovery period after the breast implant revision is much easier than it was after the original breast augmentation surgery. After the surgery, there will be stitches on your incisions and dressings on your incisions, and you will need to wear a support bra or a compression garment for some time. Right after the surgery, it is normal to experience some pain and discomfort. You will also experience some bruising and swelling. Pain and discomfort will improve after a few days. Until then, pain can be successfully controlled by pain medications. Small, thin tubes may be temporarily placed under the skin to drain excess fluid and blood. You will be able to walk on the day of the surgery, but you will need several days before returning to your other daily activities. Your stitches will be removed after a week.

Later

The swelling and bruising will improve significantly past the first week. The most of the remaining swelling and bruising will be gone within the first month after the surgery. Most of the patients are able to return to work 7 to 10 days after the surgery. It may require more or less time depending on the type of activities you perform. You may have altered sensation around the incisions, which should be temporary. 6-8 weeks post operation, the scars will start to fade, and you will be able to return to most of your regular activities including exercise. The exact duration of the recovery depends on the procedure as well as the patient. If you have a combined breast implant revision with breast lift, your recovery period can be longer while if your implants are removed without placing new ones, it can be shorter.

Guidance & Support

A wellbeing assistant will visit you in your hotel room every day during the first week after the surgery. We will expect you to be in our clinic for your post-op examination 1 week after the surgery, before you leave Istanbul. We will still follow your recovery process while you are away. What to Do Before Breast Implant Revision?

During your online consultation, your surgeon will ask you questions about your medical history. It is of critical importance that you give your doctor every detail, from the conditions you might have to the operations you had in the past, to the medications and supplements you use, to minimize the risks the surgery could bring. They will also conduct a physical examination. Your surgeon will inform you about everything you need to know before the surgery.   • Stop smoking at least 2 weeks before the surgery.

What to Do After Breast Implant Revision?

  • Do not smoke for at least 2 weeks after the surgery.
  • Do not take non-steroidal anti-inflammatory medications after the surgery until your surgeon approves using them.
  • Care for the surgical sites and drains.

  • During the first week after the surgery, sleep with your head and back elevated to reduce swelling, using pillows or sleeping in a recliner chair.
  • Avoid strenuous activities and exercises for several weeks.
  • If you experience chest pains, unusual heartbeats, or shortness of breath, seek medical attention immediately.
  • Wear the support bra or the compression garment as prescribed by your surgeon.

Even when there is no problem, you may choose to have a breast implant revision surgery if you are not happy with the results of your primary breast augmentation surgery. You can be unsatisfied with the results, which is very understandable. The results may not have matched your expectations or your desires could have been changed. Whatever the reason might be, if you are healthy and have realistic expectations, you can have a breast implant revision where your breast implants will be replaced with new ones. During the surgery, the incision scars left from your original breast augmentation surgery will be used, so no new scars will be formed. Your surgeon may manipulate the implant pockets to make them better fit the new ones: if your new implants are larger, the implants pockets may be enlarged and if they are smaller, the surgeon may need to reduce their size.
In these cases, the hardened scar tissue capsule that is causing this problem should be removed. If you have developed capsular contracture once, the chances of having another are higher than the average. However, this doesn’t mean that you are going to have it again. If you decide to have a capsulectomy (removal of this scar tissue capsule), you have two options: you may choose to have implants placed after the capsulectomy, or you may choose not to have implants. If you choose not to have implants anymore, during your surgery, the implant and the tissue capsule will be removed. On the other hand, if you still want to have implants, after the removal of the old implant with the capsule, a new implant will be placed.
This is absolutely normal. Results of the breast augmentation are long-lasting. However, aging, pregnancies, and weight fluctuations change the appearance of the breasts. But don’t worry! If you intend to have a breast implant revision surgery, the effects of time, gravity, your pregnancy and possible weight changes can be reversed. Because you mentioned “years and pregnancy”, you will probably need a breast lift, which can be combined with your breast implant revision surgery, which means, the excess skin on your breasts will be removed and your nipples and areolas will be repositioned. This is also a chance to change the size and shape of your breast implants. If you are considering that another type of breast implant would be better for you, this is your chance to get them! After the procedure, you will have “lifted”, youthful, and aesthetically pleasing breasts.
