Anesthesia
First of all, you will receive anesthesia medications. Depending on the particular case, the surgery can be performed using either general anesthesia, or local anesthesia combined with a sedative.
Implants
Before your surgery begins, your surgeon will measure your legs in order to determine which implant size, shape and position will best meet your desired goals and then customize the right implant for the overall appearance you want to achieve. Calf implants generally contain silicone which is durable and rubberlike material. They have a cigar-like shape and they are available in different sizes, thickness, lengths, and widths.
Surgery
After administering anesthesia, your surgeon will make an incision along the natural creases behind your knees and cut through the skin and fascia which covering the gastrocnemius muscle and then make a pocket for a calf implant between the muscle and fascia. The calf implant will augment the muscles forming the calves. Depending on your goals, a second implant might be placed alongside the first one to give more definition and shape. After the placement of an implant, the surgeon will carefully close the incisions with sutures. A compressive bandage will be placed in the area in order to reduce swelling. Following the surgery, your calf implant will become enclosed by scar tissue and this natural process helps the implant stay intact. Therefore, it can remain its place for years. The surgery generally takes about an hour.

You may need to take at least a few days off work. You may be given compression bandages and you need to keep them for two weeks. You may experience soreness in your legs following the surgery and you may need assistance while walking. Within a week, you will be able to walk without an assistance. You may experience bruising at the incision areas and your calves will be swollen. You may also experience discomfort and redness within the first few days, but they will subside quickly. You can be given pain medication to reduce discomfort. You should be in bed in the first few days after the operation and you should keep your legs elevated in order to minimize swelling and discomfort. Your caregiver should be with you during the initial period to assist when you need to go to the bathroom or get something. You may be instructed to use crutches for a period of time. You can resume normal daily activities within one or two weeks. Most of the patients are able to resume normal level daily activities within one or two weeks and most patients can return to work within two weeks following the calf implant surgery. For two weeks, you should avoid physical exercise and stay off feet. Any bruising and inflammation will subside eventually in a couple of weeks. You can return to normal level walking within two weeks. You should avoid effortful physical activities such as weightlifting, running and biking for one or two months. You will probably be able to return your normal exercise routine within 4-8 weeks, but this time period might be different for every patient. Your surgeon will tell you when you are ready according to your healing rate. Your results will be visible immediately after surgery and they will be long term. However, the silicone implants may degrade from natural wear and tear and once this happens you may need to change your implants and redo the surgery.
